In Florida, a Seller is obligated to disclose to a Buyer all known facts that materially affect the value of the property being sold and that are not readily observable. This disclosure statement is designed to assist the Seller in complying with the disclosure requirements under Florida law and to assist the Buyer in evaluating the property being considered. This form should be used in conjunction with the Mold Addendum. |

Items marked "(p.o.c.)" were paid outside the closing; they are shown here for informational purposes and are not included in the totals. This form is issued at closing for both the buyer and seller to clearly show all costs associated with closing. |

Every purchaser of any interest in residential real property on which a residential dwelling was built prior to 1978 is notified that such property may present exposure to lead from lead-based paint that may place young children at risk of developing lead poisoning. |

The LBP brochure is required to be provided by law on properties built prior to 1978. Purchaser or tenant must receive the brochure. |
